ELKINS-RANDOLPH COUNTY YMCA PERMISSION FORM FOR A MINOR ATHLETE TO TRAVEL TO A COMPETITION WITH A NONCUSTODIAL PARENT OR GUARDIAN
* A new form must be completed for each event and provided to a RAYS coach.
I give permission for my child, ____________________________________________, and grant an exception to the Minor Athlete Abuse Prevention Policy, to travel with ______________________________________________________ (name of adult), to attend the _________________________________________ (name of competition)
from ___________________ to ___________________ (dates of travel to competition).
I acknowledge that minor athletes cannot share a hotel room, sleeping arrangement or other overnight lodging location with coaches or non-custodial parents or guardians at any time. I further acknowledge that this written permission is valid only for the dates and location specified herein. I have carefully read this agreement and understand its contents. I am aware that signing this form implies an assumption of risk, waiver and a release of liability and I sign it voluntarily.
